There are 3 ways for people to participate in the MDGA membership and newsletter survey:

 

First, here is a link for the MDGA Member Survey, on which you can click in and complete on-line.

Second, this same link is also pasted to an email to the MDGA listserve and the MDGA Facebook page if you have those connections. 

 

Third, a PDF document of the survey that can be printed is also available.  Click Here. You can fill this out and either email it to me or fill it out as a hard copy and mail it to me.
